Before your visit
  • Complete intake forms at least 24 hours in advance
  • Upload any relevant prior records
  • Ensure you have a private, quiet space for your telehealth visit
  • Test your camera and microphone beforehand
After your visit
  • View your visit summary and Blueprint in the portal
  • Complete any follow-up questionnaires if requested
  • Access your superbill for OON reimbursement submission
  • No further action needed unless instructed

Core members: secure messaging with staff-triaged response. Signature members: direct physician response. Messaging is for care coordination only — not clinical consultations.

Through the patient portal, you can…

  • Complete intake questionnaires and consent forms

  • Upload relevant medical records (optional but recommended)

  • Attend your scheduled telehealth visit

  • View visit summaries and clinical documentation

  • Receive administrative messages related to your appointment

  • Access invoices and receipts for self-pay services

Before Your Visit

  • Complete all required intake forms at least 24 hours before your appointment

  • Upload any relevant records if available (optional)

  • Ensure you can access video and audio from a private location

After Your Visit

  • Visit summaries and documentation will be available in the portal

  • Administrative follow-up (if needed) will occur through secure messaging

  • No further action is required unless instructed during your visit
